Melasma is primarily an epidermal pigmentation characterised by tan or brown patches, most commonly on the face. Melasma can occur during pregnancy. It can also be triggered by oral contraceptives and is common in peri-menopausal women.

The combination of age, pregnancy, oral contraceptives, and sun damage are the most important in the development of Melasma. In hormonal-induced Melasma, once childbirth has occurred or the use of oral contraceptives has been discontinued, the condition may partially dissipate on its own. If the cause is non-hormonal, using sunscreen and avoiding sunlight exposure will limit the severity of the condition.

Pigmentation is the colouring of a person’s skin. In a healthy person, the skin colour appears normal and even toned. Overexposure to the sun, stress, hormonal fluctuations, aging or trauma to the skin (burns and cuts) can cause the skin to change colour. When the skin becomes darker, it is called hyperpigmentation. When it becomes lighter, it is called hypopigmentation.

Hyperpigmentation irregularities are common, occurring when pigment producing cells called melanocytes become overactive, producing too much melanin.

Lentigines. Lentigines are flat brown epidermal lesions. They may be any size from 5-20mm and have a clearly defined edge. They typically occur on the sun exposed areas such as the face and neck, dorsum of the hands and forearms and shoulders.

Dermal Macules. These are typically seen as clusters of discrete black spots in the cheeks of females. They may appear in the 20-30 year age group and tend to darken with each pregnancy.

Acne scars, chickenpox scars, and trauma scars are unpleasant reminders of past skin conditions, illnesses or accidents. Acne scarring is a frequent concern for many of our patients.
